To get ready for your appointment, please do the following:

1. Download and print the five documents below.

2. Fill out the Intake Form and Symptom Survey completely.

3. Read and agree to the Policies and Privacy Practices.

4. Read and sign the Medicare Private Contract if applicable.

5. Follow additional instructions in the Policies document. 

6. Bring the filled out forms as well as the supplements and medications you take to your visit.

7. Bring copies of pertinent and recent lab or imaging reports for your file with you to your appointment.
